Advanced Fertility Treatments

  • Blastocyst culture: In IVF/ICSI, embryos are transferred in mother’s uterus three days after the fertilisation/egg retrieval. However, in blastocyst culture, the embryo is allowed to grow in the laboratory up to day 5. When this occurs, the pool of embryos to choose from reduces, leaving behind healthy embryos that have better survival in the uterus. This gives us a better chance at a successful pregnancy. However, various circumstances determine whether the embryo transfer is ideal on day 3 or day 5. It varies from patient to patient. Wherever the option of blastocyst appears suitable, it is discussed with the couple before planning embryo transfer or embryo freezing.

 

  • Granulocyte colony stimulating factor (G-CSF): G-CSF is a type of growth factor which promotes the regenerative activity of the endometrial cells in the uterus. G-CSF is usually advised in couples who have failed multiple IVF treatments or those with thin endometrium, as it improves the chances of implantation of the embryo and maintenance of pregnancy. It can be instilled inside the uterus or given as a subcutaneous injection.

 

  • Platelet-rich Plasma (PRP): PRP is a concentrated blood product rich in platelets which secrete Platelet Derived Growth factors, used in IVF or IUI procedures to increase the chances of pregnancy. PRP can be used as intrauterine instillation to improve endometrial receptivity, endometrial blood flow & thickness.

 

  • Hysteroscopy with subendometrial PRP instillation: In this procedure, PRP is administered into the subendometrial space using a needle through the hysteroscope’s operating channel. Subendometrial space is the region beneath the lining of the uterus where the embryo implants itself. The growth factors present in PRP help in improving blood supply thereby increasing the thickness & receptivity for the embryo, optimising the chances of positive pregnancy.

 

  • Ovarian Rejuvenation: Ovarian rejuvenation is a process which involves stimulating ovarian function for better growth of primordial follicles and eggs. This is achieved by administering PRP into the ovaries in the subcortical plane where primordial follicles reside. This procedure is done under ultrasound guidance with transvaginal probes. This is advised in selective patients with low ovarian reserve, poor quality of eggs, Premature Ovarian Failure/ Insufficiency.
